Preparing for a Visit
New patients should allow 2 hours for their initial complete appointment.
Existing patients should allow 1 1/2 hours for complete appointments.

If you do not want to pre-register online, you may download the following forms and fill them out prior to your appointment. All patients will need to fill out a medical history questionnaire at their annual exam. The demographic data and registration information should be filled out before your initial exam and any time this data changes.

If you are unable to register online or print the forms, please bring a list of all medications, as well as the dosage and instructions, with you to your visit, as well as written copy of allergies, past medical history, surgeries and any other pertinent information.
If you are a contact lens wearer, bring a pair of your current contact lenses, as well as the prescription or packaging. Bring your current glasses, even if you do not like them.
